Event Scheduling Form
Please be sure to print both pages of the form; submit the form to April Toncray.
Upon approval, your event will be posted on the church calendar.
Approval or denial of the application will take up to five (5) business days.
The form was approved at church conference and REQUIRES that use of the kitchen must meet qualifications of the
Commonwealth of Virginia Department of Health regulations.
Current copies of "Food Handler's Cards" must be attached to the Event Scheduling Form.
Food Handler's cards do not guarantee kitchen usage approval.
Request forms WITHOUT Food Handler's card will be DENIED use of the kitchen.
